Nurse Practitioner

Part time

      As a Nurse Practitioner on the Palliative and Hospice Care Team, you will provide Palliative and Hospice Care to patients in the hospital setting and/or skilled nursing facility and/or home setting. This excellent opportunity affords a flexible, autonomous role bringing enormous satisfaction in the care and comfort of our aging population. Additionally you will build meaningful relationships with the residents and their families.

Our Nurse Practitioners collaborate with the Executive Director, Medical Director and Hospice Clinical Director to ensure that quality Palliative and Hospice Care is being provided to our patients.

Required Qualifications:
      • Current, unrestricted NP license in the State of IN
      • A clinical background in adult, family or geriatric specialties
      • DEA or ability to obtain Prescriptive authority or ability to obtain
      • Minimum 1 year NP experience
      • Ability to work autonomously
      • Reliable transportation and insurance
      • Ability to travel throughout our service area
      • Intermediate level of proficiency in PC based word processing including Microsoft Word and Outlook
Preferred Qualifications:

Hospice Clinical Supervisor


Provides over-all supervision related to the daily operations of the Hospice Home Care and Hospice Inpatient Center and coordinates and delegates the implementation of the plan of care for each patient and serves as the Hospice program Patient Care Coordinator to assure coordination of care by performing the following duties personally or through subordinate staff.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Demonstrates behaviors that promotes and supports a philosophy of excellence in customer relations and which support the mission, vision, and values of the agencies.

Establishes and maintains effective interpersonal relationships with internal and external customers.  Functions as a team member at all times.  Insures that interdisciplinary care is provided.

Participates in assuring that the Hospice Program is in compliance with state and federal regulations and accreditation standards.

Keeps the Director of Clinical Services apprised of all issues that may have an impact on the operations of the Hospice Program or agencies.

Detects, reports, and corrects work activities that may potentially be considered inappropriate, illegal or fraudulent.

Oversee orientation of new staff and assures that training and education needs of staff are met in accordance to agency’s policies and procedures, as well as, all state and federal regulations including performance improvement.

Coordinates Hospice Program activities and the work assignments of staff members.

Evaluates activities of the Hospice Program to ensure patient care, staff relations, and efficiency of service based on agency goals and objectives.

Assures that appropriate hospice service policies are developed and implemented based on identified goals.

Responsibility for insuring continuity of care through coordination of nursing, social work, and other team member schedules.

Participates in supervisor on-call rotation

Education and/or Experience:

Graduation from an approved school of professional nursing whose program is accredited by the National League of Nursing.  Minimum two years of medical/surgical experience in a hospital setting or home care experience.  Previous hospice experience and/or demonstrates understanding and support of the Hospice philosophy and concepts. Exhibits sensitivity to issues of loss and grief.

Hospice RN

Evenings, Full time

At the VNA Hospice Center, a 12 bed inpatient facility

          The Registered Professional Nurse serves as a case coordinator by providing, arranging for, teaching and supervising the health care of hospice patients.  The nurse appraises patient and family care needs and coordinates services with the interdisciplinary team.
          Assumes primary responsibility for nursing care of the patient in conjunction with physician’s orders and IDT.
          Understands and can interpret to patient and family the philosophy, acceptance, admission, recertification, and discharge policies for all agency programs and levels of care.
          Observes, evaluates, and reports to physician patient’s physical and emotional condition, reaction to drugs and/or treatments.  Interprets to physician social and physical factors in the environment that affect patient’s care.
          Identifies nursing problems and establishes plan of care based on mutual goals.
          Makes ongoing re-assessment of patient/family’s current status and initiates changes in plan of care as necessary.
          Evaluates need for additional agency and/or community resources and makes appropriate referrals including caregivers at risk for pathological grief.
          Coordinates and/or implements other agency professional services for his/her patients and families.  Prioritizes needs with the members of IDT.
          Instructs and supervises HHA/LPN’s.
          Consults with and educates the patient and family regarding the disease process, self-care techniques, prevention strategies, end of life care and process to deal with issues of ethical concern.
          Helps patient and family to adjust positively to physical, mental and social limitations.  Helps family accept and cope with responsibilities for providing care. 
          Respects the right of the patients to maintain reasonable control of their lives and retain as much decision-making authority as possible.
          Helps to achieve and maintain continuity of patient care by planning and exchanging information with other health and social agencies as well as by intra-agency exchange of information.
          Utilizes the team approach to achieve and maintain continuity and comprehensiveness of patient/family care.
          Serves as an auxiliary member of IDT and attends IDT meetings when requested to do so.  Participates in re-certification process per agency policy.
          Abides by shift schedule. Performs other duties as assigned.
          Provides specialized hospice training to other staff, family members, and informal caregivers to insure adequate care.
          Must be currently licensed as a registered professional nurse by the state of Indiana.
          Diploma must be from an approved school of professional nursing. Must maintain current CPR certification.
          Minimum of one year of medical/surgical experience in a hospital setting, nursing home, ECF, or home care.  Previous hospice experience and/or demonstrates understanding and support of the hospice philosophy and concept preferred. Demonstrates comprehensive knowledge and skill in current nursing practice.
          Must hold and maintain a valid driver’s license, auto insurance, and professional liability.
          Exhibits knowledge of the concept of a interdisciplinary team and a working knowledge of the development of plan for patient care and the ability to participate in them as well as using the case management approach.
          Functions independently and appropriately.

Clinical Relations Liaison RN



The Clinical Relations Liaison will assist with the marketing of the agency and will assist in developing and maintaining referral sources for the agency.  This position also serves as a liaison between current referral sources and the agency. This position is responsible to assist in the growth of the agency and to increase admissions to the agency.


· Assists with Marketing Activities:

· Builds and maintains relationships with referral sources by calling on potential and current referral sources, including but not limited to, physicians’ offices, extended care facilities, hospitals, community organizations, and other appropriate community resources.  Presents the agencies in a positive way at all times.

· Promotes the Agencies through making calls and visits, delivering new physician packets, celebrating Doctor’s Day, Social Worker’s Day, Doctor of the Year, delivering tokens of appreciation, and providing education.

· Schedules and coordinates in-services as requested to potential or current referral sources to explain the Agency’s services and provide education

· Participates in enhancing marketing activities by making recommendations on current marketing programs, identifying needs for marketing materials/resources, suggesting new marketing strategies, and identifying new potential referral sources or market segments.

· Attends and assists with staffing functions as required including weekends and evenings.

Community Relations Activities:

· Serves as a liaison between the Agencies and the referral sources.

· Assists in researching problems or concerns and following through to resolution.

· Notifies supervisor of quality improvement or other needs identified by customers.

· Works as liaison between referral sources and agency for patient admissions.

Job Requirements

· Previous experience in marketing or related field preferred.

· Excellent oral and written communication skills.

· Excellent presentation skills.

· Must be computer literate.  Experience in database management, Power Point, desktop publishing, and Excel preferred.

· Must be able to work a flexible schedule, including evenings and weekends as needed.

· Demonstrates ability to take initiative.

· Ability to work independently and be extremely organized.

· Must have pleasant personality and professional appearance.

· Ability to develop effective working relationships with all staff members.

Hospice Social Worker

    The Visiting Nurse Association Northwest Indiana is seeking a Hospice Social Worker (full-time) to provide direct supportive services to terminally ill patients and their families at our in-patient Hospice Center and Patient Homes.
    The Hospice Social Worker works in conjunction with the Hospice interdisciplinary group (IDT) to provide direct supportive services to terminally ill patients and their families, and to consult with and support other agency personnel .The social worker will provide assessments of psychosocial problems and define goals and interventions to provide excellent end of life care.
The Hospice Social Worker:
    Is a member of an interdisciplinary team (IDT) and provides social services in accordance with interdisciplinary plan of treatment.
    Participates in the preparation, evaluation, and execution of the interdisciplinary plan of care to assure that the highest quality of care is given to those served.
    Recognizes and utilizes community and family resources to assist the patient and family with end of life care needs.
    Develops the plan of care with the IDT to deal with personal, financial, and environmental difficulties that may interfere with care.
    Assists with funeral planning.
    Keeps abreast of social work trends and knowledge.
    Participates in education both internally and externally to promote professional growth and understanding of hospice.
    Maintains continuity of patient care by liaising with other health care professionals.
    Attends IDT conferences and care plan meetings.
    Arranges for extended care placement or respite placement when necessary.
    Demonstrates flexibility with scheduling.
    Must possess a Master's Degree from a school of social work accredited by the Council on Social Work Education.
    Must be licensed by the State of Indiana.
