Community Outreach Coordinator

Full Time

This position is responsible for coordinating the various VNA community outreach activities and services, including but not limited to Meals on Wheels and Lifeline emergency response services. The position reports directly to the Office Manager.

TYPICAL TASKS / DUTIES

  • Create and implement a community outreach plan, including the development, marketing and administration of the VNA Programs and Services that complement the VNA Hospice and Palliative Care Programs.
  • Execute marketing of all VNA outreach services, coordinating as appropriate with other VNA business development initiatives.
  • Assure the effective delivery of all VNA outreach services, training and monitoring the performance of assigned employees and volunteers.
  • Organize and participate at community outreach activities to educate the broader community about the VNA, our activities and projects.
  • Respond to public and community inquiries and brief Authority staff on matters of public interest and concern. • Participate in construction status meetings.
  • Oversee the maintenance of a community database of interested parties and issues.
  • Perform miscellaneous projects as assigned.

KNOWLEDGE, SKILLS AND ABILITIES

  • Knowledge and experience in working with diverse communities in Northwest Indiana
  • Exceptional interpersonal skills
  • Strong writing and oral presentation skills
  • Experience in facilitating public meetings and other public forums
  • Exceptional organizational and planning skills
  • Knowledge and command of Microsoft Office (including Power Point and Access)
  • Available to attend evening and weekend community events and meetings
  • Comfort and self-confidence to work independently with minimal supervision.

EDUCATION AND EXPERIENCE

  • Candidates should have a minimum of three years in marketing, program/service administration community relations, public affairs or related field.
  • Experience within a non-profit social service agency/organization is preferred.
  • A minimum of an Associate Degree required, with a Bachelor’s Degree preferred.

PHYSICAL DEMANDS

  • The physical demands described here is representative of those an employee encounters while performing this job: The position is regularly required to extensively make use of close vision to read correspondence and prepare reports and other appropriate communications.
  • S/he must also able to be transient and hold a valid driver’s license to attend community meetings and transport information and visit customers or production/delivery sites.

  • Many meetings and events require evening and weekend attendance

WORK ENVIRONMENT

  • The work environment described here is representative of those an employee encounters while performing this job: The position typically works indoors in an air-conditioned office, with a mixture of natural, incandescent and florescent light. Typical noise levels are muted by acoustic ceilings, carpets, and sound-deadening wall panels. On occasion, the position is called upon to work outside of the office at a public meeting or event or when dealing with clients Some of these occur indoors; while others occur outside.

Receptionist and Client Services Assistant

Full Time

The Receptionist and Client Services Assistant is the first point of contact and provides administrative office support in a team environment. He or she is a role model for professionalism, customer service and compassion for those telephoning or visiting VNA offices.

Responsibilities:

  • Provides client intake services in a professional, caring, and compassionate manner to caregivers, Meals on Wheels recipients, and clients for other programs and services
  • Obtains accurate client demographic, financial and insurance information
  • Answers general questions about VNA services while communicating with caregivers and recipients
  • Enrolls clients in Lifeline emergency response system and assists with enrollment of Meals on Wheels clients.
  • Assists in projects as requested by staff such as mailings, labeling and preparing presentations
  • Assists with data tracking and statistical reporting as needed
  • Receives and distributes incoming mail
  • Oversees the operation of office machines, including copy machine, printers mail meter, phone system and outgoing mail.
  • Provides clerical support for the Referral Team
  • Trains office volunteers when required
  • Performs such other duties as may be assigned from time to time

Qualifications:

  • High school diploma or GED required, Associate Degree preferred
  • Minimum 1 year administrative support
  • Excellent phone skills and able to answer multi line phone system
  • Strong communication skills (oral & written)
  • Proficient in the use of computer applications MS Word, Excel & Outlook
  • Must have experience and knowledge of database work
  • Excellent organizational skills and must be detail oriented
  • Bilingual in English and Spanish helpful but not required

The Licensed Practical Nurse performs designated nursing functions under the direction of the Registered Nurse. The activities of the LPN are based on his/her educational preparation and experience.

Hospice Aide (HA) is a member of the Hospice Aide Department and the Hospice Interdisciplinary Team who has been trained in the skills that enable her/him to provide personal care for individuals in their residence or at the Hospice Center, under the supervision of a registered nurse.

Must have availability for orientation.

HOUSEKEEPING - VNA HOSPICE CENTER

VNA of NWI is seeking a part-time housekeeper to clean our Hospice Center and Administrative offices.

Part-time/Days Shifts (20 hours a week with weekend rotation/must have flexibility to work additional hours as needed)

POSITION SUMMARY

Under the direction of the General Direction of the CEO and Direct direction of the Office Manager performs all the functions necessary for the janitorial upkeep of all the buildings. Establishes and maintains excellent working relationships with all internal departments and outside vendors.

QUALIFICATIONS: High school diploma or GED equivalent.

WORK EXPERIENCE, SKILLS, AND ABILITIES:

  • Previous janitorial experience preferred.
  • Must have ability to organize/prioritize daily activities.
  • Must have ability to follow through with assignments in a timely manner.
  • Must be able to work a flexible work schedule and participate in weekend and holiday rotation as needed.

PHYSICAL REQUIREMENTS:

The physical activities of the position involve balancing, climbing, crouching, kneeling, lifting, fingering, grasping, pulling, pushing, reaching, repetitive movement, sitting, standing, and stooping. The sensory and communicative activities essential to the performance of this position include: feeling, hearing, seeing and speaking. An individual in this position must be able to lift, carry, push and pull objects in excess of 40 pounds. An individual in this position will be exposed to inside and outside environmental conditions. Inability to perform the physical requirements of this position would pose a significant, if not direct, threat to departmental operations.

LPN Hospice

SUMMARY

  • Graduate of a state accredited school of practical nursing.
  • Must hold and maintain current LPN licensure in the State of Indiana.
  • Evidence of experience in related health care field.
  • Must hold and maintain a valid driver’s license, auto insurance, and professional liability.

FUNCTIONS

  • Assists the RN and/or physician in performing specialized duties related to end-of-life care.
  • Implements and continues the appropriate teaching in cooperation with the Registered Nurse.
  • Observes the patient for changes in their condition and reports these changes to the appropriate physician and registered nurse.
  • Records visits to patients per agency policy, including other pertinent information about patient and family.
  • Follows care plans as developed by the Registered Nurse and contributes data to the RN in the development of the plan of care and assists in carrying out the plan.

CNA/HHA Hospice

Full-time with On-Call Rotation

Must be able to travel to all service areas: Porter, Lake, Jasper, Starke and LaPorte Counties. Provides personal care to hospice patients at the VNA Hospice Center and patients Home.

QUALIFICATIONS:

  • Prefer minimum of 6 months as a CNA or HHA.
  • Current CNAs will receive training and certification as a HHA upon hire.
  • Must have and maintain valid driver's license, auto insurance. Must maintain current CPR certification.
  • Must be computer literate.
