Hospice Clinical Supervisor RN

Participates in supervisor on call rotation

The Hospice Clinical Supervisor is responsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Supervisor is responsible for ensuring that care and services are delivered appropriately and for supervision of clinical personnel.

Essential Duties and Responsibilities include the following. Other duties may be assigned:

  • Demonstrates behaviors that promotes and supports a philosophy of excellence in customer relations and which support the mission, vision, and values of the agencies.

  • Establishes and maintains effective interpersonal relationships with internal and external customers. Functions as a team member at all times. Insures that interdisciplinary care is provided.

  • Participates in assuring that the Hospice Program is in compliance with state and federal regulations and accreditation standards.

  • Keeps the Director of Clinical Services apprised of all issues that may have an impact on the operations of the Hospice Program or agencies.

  • Detects, reports, and corrects work activities that may potentially be considered inappropriate, illegal or fraudulent.

  • Oversee orientation of new staff and assures that training and education needs of staff are met in accordance to agency’s policies and procedures, as well as, all state and federal regulations including performance improvement.

  • Coordinates Hospice Program activities and the work assignments of staff members.

  • Evaluates activities of the Hospice Program to ensure patient care, staff relations, and efficiency of service based on agency goals and objectives.

  • Assures that appropriate hospice service policies are developed and implemented based on identified goals.

  • Responsibility for insuring continuity of care through coordination of nursing, social work, and other team member schedules.

  • Participates in supervisor on-call rotation

Education and Experience:

  • Registered nurse with current licensure to practice professional nursing in the state.

  • Graduation from an approved school of professional nursing whose program is accredited by the National League of Nursing.

  • Previous experience in hospice/home care setting and with (2) years management or supervisory experience. Prove ability to work within an interdisciplinary setting.

Hospice Volunteer Coordinator

Seeking a Full-time, Volunteer Coordinator

SUMMARY:

The Hospice Volunteer Coordinator plans, coordinates, and implements the volunteer program for Hospice, including retention, marketing and scheduling of volunteers.

Program Management Activities:

  • Communicates effectively with staff and volunteers to ensure that volunteers are utilized as an integral part of the Hospice team.
  • Develops, implements, and evaluates programs for recruiting and training volunteers.
  • Interviews, screens, and evaluates all volunteer applicants.
  • Responsible for the coordination of orientation and training of volunteers with input and help for the Hospice team, and conducts the training program.
  • Responsible for creating regularly scheduled monthly volunteer newsletters and distributes for mailing.
  • Assists Human Resource Department in maintain an up-to-date personnel file including current medical file on each volunteer ensuring compliance with all state and federal regulations.
  • Notifies volunteers in a timely manner when documentation is required.
  • Develops and implements volunteer policies and procedures which complement the agency and promote volunteer commitment.
  • Assists Accounting Department who provides annual reports for volunteer service by collecting data and identifying goals. Establishes and develops systems for data collection.
  • Represents agency in community activities and organizations as appropriate.
Volunteer Assignment and Supervision Activities:
  • Provides on-going meetings and educational activities for volunteers for the purpose of support, problem-solving, and continuing education.
  • Assigns volunteers to patients and clerical/administrative functions in coordination with staff.
  • Ensures effective utilization of volunteers by assigning volunteers to meet programs needs with consideration of individual skills, knowledge and desire.
  • Participates in team conference/Interdisciplinary Team (IDT) and establishes a volunteer plan of care for each patient/family.
  • Maintains a regular communication linkage with volunteers by monitoring their placement and by providing individual conferences, meetings, and special programs.
  • Assists in supervising volunteers in all special projects and fund raising efforts, i.e. memorial services, bereavement reunion dinner, annual volunteer recognition dinner, etc.

Hospice Aide (HA) is a member of the Hospice Aide Department and the Hospice Interdisciplinary Team who has been trained in the skills that enable her/him to provide personal care for individuals in their residence or at the Hospice Center, under the supervision of a registered nurse.

Must have availability for orientation.

Office Manager

Full Time

SUMMARY:
The Office Manager position is responsible for overall front office activities of the Hospice Organization, including the reception area, mail, large purchasing requests and facilities. Also responsible for directing and coordinating office services and related activities, including developing and supervising programs for the maximum utilization of services and equipment. Responsible for arranging internal office moves and providing arrangements for office meetings. Independent judgment is required to plan, prioritize and organize diversified workload.

ESSENTIAL FUNCTIONS:

  • Provides secretarial support for the Executive Director and Management Team:
  • Manages the reception area to ensure effective telephone and mail communications both internally and externally to maintain professional image.
  • Supervises and coordinates overall administrative activities for the Office Administration Department.
  • Supervises the maintenance and alteration of office areas and equipment, as well as layout arrangement and housekeeping of office facilities.
  • Negotiates the purchase of office supplies and furniture, office equipment, etc., for the entire staff in accordance with company purchasing and budgetary restrictions.
  • Responsible for the facilities day-to-day operations.
  • Performs office duties including copying, faxing, typing general correspondence, letters, and reports.
  • Composes letters and memos as requested.
  • Supports the Executive Director in building and maintaining relationships with other professional organizations, the community, and internal departments.
  • Makes travel and hotel arrangements as requested.
  • Schedules conference rooms for meetings.
  • Sets up teleconferences as requested.
  • Maintains department filing system, contracts, etc.
  • Maintains a tickler file of upcoming events, contract renewals, projects, etc.
  • Maintains Administration manuals to meet regulatory compliance.
  • Coordinates overall administrative activities for the Office Administration.
  • Performs other duties as assigned.

Provides support for the Board of Directors and Committees:

  • Serves as a communication liaison between the Executive Director and the Board members.
  • Responds to requests for information in a timely manner.
  • Coordinates Board Meeting logistics, such as scheduling conference room, equipment, ordering/getting food and beverages, etc.
  • Attends Board Meetings and takes minutes.
  • Types and distributes Board Meeting agenda and minutes.
  • Maintains accurate database of Board Members and their contact information.
  • Maintains and updates Board manuals.

  • Performs other tasks as assigned.

Responsible for preparing, examining, analyzing, negotiating, and revising contracts that involve the purchase or sale of goods or services such as equipment, materials, supplies, or products.

  • Oversee the preparation and revision of contracts that involve the purchase of goods and services.
  • Oversee proposal planning and administrations of contracts along with storing and distribution.
  • Maintain detailed and organized files.
  • Maintain an audit file for each contract which will include original contract, all correspondence, changes/deviations, amendments, clarifications, payment schedule termination and renewal dates.
  • Track payments and renewal deadlines communicate with appropriate team members.

QUALIFICATIONS:

  • Education: High School diploma or GED equivalent required.

  • Must be computer literate. Experience in Microsoft Word or Word Perfect required, Power Point, Excel

  • Demonstrates ability to take initiative.
  • Ability to work independently or in a team setting and be extremely organized.
  • Must have pleasant personality and professional appearance.
  • Ability to develop effective working relationships with Board, management, and staff members.
  • Ability to maintain confidential information and remain professional at all times.
  • Must be flexible and adapt easily to a changing environment.
  • Must have and maintain current driver’s license and automobile insurance.
  • This is a full-time position, and hours of work and days are Monday through Friday, 8:00 a.m. to 4:30 p.m., with some flexibility to cover board and committee meetings.
