POSITION SUMMARY

The Registered Nurse serves as a case coordinator by providing, arranging for, teaching and supervising the health care of hospice patients. The nurse appraises patient and family care needs and coordinates services with the interdisciplinary team.

The Licensed Practical Nurse performs designated nursing functions under the direction of the Registered Nurse. The activities of the LPN are based on his/her educational preparation and experience.

Hospice Aide (HA) is a member of the Hospice Aide Department and the Hospice Interdisciplinary Team who has been trained in the skills that enable her/him to provide personal care for individuals in their residence or at the Hospice Center, under the supervision of a registered nurse.

Must have availability for orientation.

Position Summary:

  • Performs consultation and admission visit to assess patient's needs and completed admission assessment and agreement documentation for VNA of NWI.
  • Possesses understanding of Hospice program and benefits.
  • Obtains orders for and procures immediate medications and needed DME/supplies.
  • Performs physical assessment of patients' condition, safety, knowledge, pain and symptoms.
  • Initiates Plan of Care
  • Instructs on disease understanding, progression, and immediate interventions.
  • Documents assessment, current medications and initiates appropriate care plans.

QUALIFICATIONS:

  • Licensure: Must have and maintain license as a registered professional nurse by the state of Indiana.
  • Diploma must be from an approved school of professional nursing.
  • Minimum of one year of medical/surgical experience in a hospital setting, nursing home, ECF, or hospic care preferred.
  • Previous hospice experience and/or demonstrates understanding and support of the hospice philosophy and concept preferred.
  • Must be sensitive to issues of loss and grief.
  • Must maintain current CPR certification.
  • Must hold and maintain a valid driver’s license, auto insurance, and professional liability.

Community Outreach Coordinator

Full Time

The Community Outreach Coordinator is responsible for coordinating the various VNA community outreach activities and services, will assist with the marketing, including but not limited to Meals on Wheels and Lifeline emergency response services. The position reports directly to the Development Director.

MAJOR FUNCTIONS

Marketing Activities include:

  • Builds and maintains relationships with referral sources by calling on potential and current referral sources, including but not limited to, physicians’ offices, extended care facilities, hospitals, community organizations, and other appropriate community resources.
  • Presents the agencies in a positive way at all times.
  • Promotes the Agency’s services via presentations and other means and responds appropriately to questions.
  • Schedules and coordinates in-services as requested to potential or current referral sources to explain the Agency’s services and provide education.
  • Provides ongoing marketing data including call logs, potential referral source list, and feedback forms.
  • Assists with department projects and events such as Christmas for the Elderly, health fairs, Hospice Month, Grief Teleconference, etc.
  • Attends and assists with staffing functions as required including weekends and evenings.

Coordination Activities:

  • Serves as agency liaison with Lifeline provider to ensure quality service.
  • Installs Lifeline Units.
  • Reviews and evaluates services provided by Lifeline.
  • Collects and analyzes data to evaluate all aspects of the program.
  • Maintains all records per agency requirements.
  • Reviews data sheets from Lifeline daily and follow up on accessing activity.
  • Reviews the billing from Lifeline for accuracy.
  • Maintains records of all installations and terminations monthly.
  • Reviews CFTE approval sheets for evaluation of funding for needy clients.
  • Keeps an inventory of 3-4 units in stock.
  • Ensures custodians have sanitized returned units.
  • Assists with backup functions to Meals on Wheels program.

Volunteer Coordination Activities:

  • Recruits, orientates, schedules and monitors volunteers.
  • Provides encouragement to and builds rapport among volunteers.
  • Assists in coordinating volunteer recognition events.
  • Perform miscellaneous projects as assigned.

QUALIFICATIONS:

  • High School diploma or GED equivalent required. Undergraduate degree in marketing preferred.
  • Previous experience in marketing or related field preferred.
  • Excellent oral and written communication skills. Excellent presentation skills.
  • Must be computer literate. Experience in database management, Power Point, desktop publishing, and Excel preferred.
  • Ability to prioritize workload and follow-through on assignments. Demonstrates ability to take initiative.
  • Ability to work independently and be extremely organized.
  • Must have pleasant personality and professional appearance.
  • Ability to develop effective working relationships with all staff members.
  • Must have and maintain current driver’s license and automobile insurance.
  • Must be able to work a flexible schedule, including evenings and weekends as needed.

EDUCATION AND EXPERIENCE

Candidates should have a minimum of three years in marketing, program/service administration community relations, public affairs or related field.

High School diploma or GED equivalent required.

Previous experience in marketing or related field preferred.
