Hospice Aides (CNA) or (HHA)

Full-time with Minimum On-Call Rotation

Coverage Areas: Porter, La Porte, Starke, Jasper and Lake Counties, mileage paid at the IRS rate.

Position Summary

Provides personal care to hospice patients at the VNA Hospice Center and patients home.

Qualifications

  • Prefer minimum of 6 months as a CNA or HHA.
  • Current CNAs will receive training and certification as a HHA upon hire.
  • Must have and maintain valid driver's license and auto insurance.
  • Must maintain current CPR certification.
  • Must be computer literate.

Position Summary

The Licensed Practical Nurse performs designated nursing functions under the direction of the Registered Nurse. The activities of the LPN are based on his/her educational preparation and experience.

Functions

  • Assists the RN and/or physician in performing specialized duties related to end-of-life care.
  • Implements and continues the appropriate teaching in cooperation with the Registered Nurse.
  • Observes the patient for changes in their condition and reports these changes to the appropriate physician and registered nurse.
  • Records visits to patients per agency policy, including other pertinent information about patient and family.
  • Follows care plans as developed by the Registered Nurse and contributes data to the RN in the development of the plan of care and assists in carrying out the plan.

Qualifications

  • License and experience.
  • Graduate of a state accredited school of practical nursing.
  • Must hold and maintain current LPN license in the State of Indiana.
  • Evidence of experience in related health care field.
  • Must hold and maintain a valid driver’s license, auto insurance, and professional liability.

Position Summary

Performs consultation and admission visit to assess patient's needs and completed admission assessment and agreement documentation for VNA of NWI.

  • Possesses understanding of Hospice program and benefits.
  • Obtains orders for and procures immediate medications and needed DME/supplies.
  • Performs physical assessment of patients' condition, safety, knowledge, pain and symptoms.
  • Initiates Plan of Care.
  • Instructs on disease understanding, progression, and immediate interventions.
  • Documents assessment, current medications and initiates appropriate care plans.

Qualifications

  • Licensure: Must have and maintain license as a registered professional nurse by the state of Indiana.
  • Diploma must be from an approved school of professional nursing. Minimum of one year of medical/surgical experience in a hospital setting, nursing home, ECF, or Hospice Care preferred. Previous hospice experience and/or demonstrates understanding and support of the hospice philosophy and concept preferred. Must be sensitive to issues of loss and grief.
  • Must maintain current CPR certification.
  • Must hold and maintain a valid driver’s license, auto insurance, and professional liability.

RN Case Manager

Job Duties

  • RN Case Manager will provide direct patient care to hospice patient case load.
  • Demonstrates comprehensive knowledge and skill in current nursing practice.
  • Exhibits knowledge of the concept of a interdisciplinary team and a working knowledge of the development of plan for patient care and the ability to participate in them as well as using the case management approach.
  • Functions independently and appropriately, and demonstrates the ability to deal effectively with the demands of the job.

Position Summary

The Registered Professional Nurse serves as a case coordinator by providing, arranging for, teaching and supervising the health care of hospice patients in their home. The nurse appraises patient and family care needs and coordinates services with the interdisciplinary team.

Part-time Human Resources Assistant (HR Certification Preferred)

Monday-Friday, 8:30am-2:00pm

Position Summary

Under the direction of the Human Resource Director, the Human Resource Assistant maintains employee and volunteer files, assists with recruitment activities including interviewing, coordinates the HHA Registry, participates in the coordination of annual events, generates reports, and provides administrative assistance to the department. Conducts new hire orientation and completes and tracks new hire events and activities until completed.

Qualifications/Education

  • High School diploma or GED equivalent required.
  • Undergraduate degree in human resources preferred.
  • Human Resource certification preferred.
  • Must have good typing and accurate data entry skills.
  • Excellent oral and written communication skills.
  • Must be computer literate.
  • Experience in Microsoft Word, database management, HRIS, Excel, and creating reports preferred.
  • Ability to prioritize workload and follow-through on assignments.
  • Demonstrates ability to take initiative.
  • Ability to work independently, be extremely organized and detail oriented.
  • Must be flexible and adapt easily to a changing environment.
  • Knowledge of Paylocity HR management is a plus.

Full-time Life Services Associate

Monday-Friday, 8:00am-4:30pm

Position Summary

The Life Services Associate assists with the daily operations of the Meals on Wheels and Lifeline programs.

Meals on Wheels Program Activities

  • Enters client information into computer for billing purposes.
  • Assigns clients to MOW route and makes route cards.
  • Prepares bags and labels weekly and arranges them in appropriate order.
  • Delivers meals when necessary.
  • Records cancelled meals.
  • Picks up signed route slips from Pines Village on weekly basis.
  • Produces weekly report regarding number of meals received per week.
  • Distribution of Meals at Pines Village daily.

Client Services Activities

  • Determines eligibility for admission to the program and evaluates appropriateness for continuation of service.
  • Completes pre-admission assessment.

Coordination Activities

  • Serves as agency liaison with Lifeline provider to ensure quality service.
  • Installs Lifeline Units.

Volunteer Coordination Activities

  • Recruits, orientates, schedules and monitors volunteers.
  • Provides encouragement to and builds rapport among volunteers.
  • Assists in coordinating volunteer recognition events.

Education and/or Experience:

  • High school diploma or equivalent.
  • One-year general office experience and previous experience supervising/organizing volunteers and volunteer activities preferred.
  • Experience working with elderly, disabled, and physically challenged individuals preferred.
  • General knowledge of nutrition and knowledge of resources for nutrition information preferred.

PRN Hospice Aide

VNA of NWI is seeking an as-needed PRN Hospice Aide for our 14 bed inpatient hospice center. Shift availability includes day, evenings, and/or nights.

Full-time, Team Care Coordinator-LPN

Monday-Friday with on-call rotation in the field making visits.

Position Summary

The Team Care Coordinator is responsible for coordinating staffing and scheduling needs to assure that patient nursing care needs are being met effectively across all required program. Schedules include daily, weekly, and monthly schedules, and encompasses clinical schedule of Hospice Center staff. This position serves as a back up to Intake/ Referral for Hospice and Palliative Care.

  • Attends weekly Interdisciplinary Group and acts as scribe for the meeting.
  • Functions in the field as an LPN to perform visits as needed for direct patient care to support fluctuation in census. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient outcomes.
  • May at times deliver consents, CTIs and other legal records or documents to families, facilities and providers as needed.
  • Will assist with Intake/ Referral workflow as needed.
  • Will call providers, place orders for medications, supplies and DME as needed. Triages incoming calls on the hospice que and refers clinical calls to the appropriate clinician or clinical supervisor in a timely and effective manner.

Qualifications

  • Graduate of state accredited school of practical nursing.
  • Must hold and maintain and maintain a current LPN license in the state of Indiana.
  • Must hold and maintain a valid driver’s license and auto insurance.
  • Maintains CPR Certification.
  • Must maintain regulatory compliance in regard to LPN supervisory visits/ competencies.
