Part-time Hospice Center RN

Must be able and willing to work 8 hour evening shifts (3pm-11:30pm) with weekend rotation in our 14 bed in-patient facility

Position Summary

The Registered Professional Nurse serves as a case coordinator by providing, arranging for, teaching and supervising the health care of hospice patients in their home. The nurse appraises patient and family care needs and coordinates services with the interdisciplinary team.

Qualifications

  • Licensure: Must have and maintain license as a registered professional nurse by the state of Indiana.
  • Minimum of Associates degree in nursing required, diploma must be from an approved school of professional nursing program. Minimum of one year of medical/surgical experience in a hospital setting, nursing home, ECF, or home care preferred. Previous hospice experience and/or demonstrates understanding and support of the hospice philosophy and concept preferred.
  • Must maintain current CPR certification.
  • Must hold and maintain a valid driver’s license, auto insurance, and professional liability.
  • Knowledge and ability
    • Demonstrates comprehensive knowledge and skill in current nursing practice.
    • Exhibits knowledge of the concept of a interdisciplinary team and a working knowledge of the development of plan for patient care and the ability to participate in them as well as using the case management approach.
    • Functions independently and appropriately, and demonstrates the ability to deal effectively with the demands of the job.
    • Ability to use and adapt to current technology, i.e. computers, laptops, pagers, infusion pumps.
    • Accepts and utilizes supervision.
    • Recognizes the needs and concerns of people and maintains constructive working relationships in dealing with them.
    • Effectively utilizes problem-solving approach to patient care and record-keeping.
    • Demonstrates understanding and support of Hospice Philosophy and concepts. Exhibits sensitivity to issues of loss and grief.
    • Ability to accept responsibility under adverse working conditions, within limits of safety, such as inclement weather conditions, and be flexible to participate in Center coverage.
    • Ability to assist patient’s ambulation; stand by assist, walker cane and crutches.
    • Ability to assist patient with range of motion exercises and other specific therapy exercise programs.
    • Ability to assist patient with transfers; manual, Hoyer lift and transfer boards.
    • Ability to evaluate your need for own support and use identified system to meet the need.

PRN (as needed) Hospice Center RN

8 hours shifts available (days 7am-3:30pm, evenings 3:00pm-11:30pm and nights 11:00pm-7:30am) must have availability for a minimum of 4 shifts a month in our 14 bed in-patient facility

Hospice Aides (CNA) or (HHA)

Full-time Days with Minimum On-Call Rotation

Coverage Areas: Porter, La Porte, Starke, Jasper and Lake Counties, mileage paid at the IRS rate.

Must be willing to go into patients homes and travel to all service areas.

Starting pay $18.50/hour

Position Summary

Provides personal care to hospice patients at the VNA Hospice Center and patients home.

Qualifications/Skills

  • Prefer minimum of 6 months as a CNA or HHA.
  • Current CNAs will receive training and certification as a HHA upon hire.
  • Must maintain current CPR certification.
  • Must hold and maintain a valid driver’s license and auto insurance.
  • Must be computer literate.
  • Proof of COVID-19 Vaccination required.

RN Case Manager

  • Documents assessment, current medications and initiates appropriate care plans.
  • Demonstrates comprehensive knowledge and skill in current nursing practice.

Hospice and Palliative Care Social Worker

Full-time

Position Summary

The Hospice and Palliative Care Social Worker works in conjunction with the Hospice and Palliative Care interdisciplinary groups (IDG) to provide direct supportive services to terminally ill patients and their families, and to consult with and support other agency personnel.

Qualifications/Education

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Master of Social Work degree from a school of social work accredited by the Council on Social Work Education; or a Baccalaureate degree in social work from an institution accredited by the Council on Social Work Education, or a baccalaureate degree in psychology, sociology, or other field related to social work and is supervised by an MSW.
  • Must be licensed by State of Indiana with an LCSW.
  • Minimum of one year of social work experience in a healthcare setting.
  • Demonstrate knowledge of Hospice philosophy, concepts, and bereavement counseling. Exhibits sensitivity to issues of loss and grief.
  • Must hold and maintain a valid driver’s license and auto insurance.
  • Proof of COVID-19 Vaccination required.
