Hospice Liaison/RN

Full-time with Minimum On-Call Rotation

Coverage Areas: Porter, La Porte, Starke, Jasper and Lake Counties, mileage paid at the IRS rate.

Position Summary

The Hospice Liaison works under the supervision of the Patient Care Manager and will primarily function as an liaison between local hospitals. Hospice Liaison is responsible for developing relationships at the designated hospitals and serving as a resource to the designated hospitals to increase awareness of the agency, increase active client count, develop referral sources with physicians and hospital staff, coordinate with intake to identify needed home care and hospice services for patients in all stages of life. Hospice Liaison provides nursing care to patients who require GIP level of care for end of life symptom management and pain control in the hospital setting.

Hospice Liaison will also assist the agency with the admission of patients in the home care setting as required to meet the needs of the agency. Hospice Admission Liaison will participate in an on-call rotation to provide the highest quality care to patients in the hospice program.

LPN

Position Summary

The Licensed Practical Nurse performs designated nursing functions under the direction of the Registered Nurse. The activities of the LPN are based on his/her educational preparation and experience.

Functions

  • Assists the RN and/or physician in performing specialized duties related to end-of-life care.
  • Implements and continues the appropriate teaching in cooperation with the Registered Nurse.
  • Observes the patient for changes in their condition and reports these changes to the appropriate physician and registered nurse.
  • Records visits to patients per agency policy, including other pertinent information about patient and family.
  • Follows care plans as developed by the Registered Nurse and contributes data to the RN in the development of the plan of care and assists in carrying out the plan.

Qualifications

  • License and experience.
  • Graduate of a state accredited school of practical nursing.
  • Must hold and maintain current LPN license in the State of Indiana.
  • Evidence of experience in related health care field.
  • Must hold and maintain a valid driver’s license, auto insurance, and professional liability.

Full-Time Account Executive

Lake, La Porte, Porter, Starke, Jasper Counties

Our Program is Expanding and We're Looking for Staff to Support Our Growth and Join Our Team!

Position Summary

The Account Executive works under the supervision of the Patient Access Director and is responsible for increasing public awareness of the agency, increasing active patient/client count, developing referral sources, representing the agency in community events and coordinating with clinical staff to identify needed services for patients in all stages of life. Provides direct education regarding hospice services and supportive services to referral sources, physicians, and the community. Serves as a resource for staff. Performs other duties as assigned.

Essential Duties and Responsibilities

Community Awareness

  • Increases public awareness of the agency and the services provided.
  • Makes contact in–person and via phone to the target population (i.e. hospitals, extended care facilities, physician offices, rehabilitation centers, community organizations, etc.) according to the specified frequency.
  • Assists with advertising according to budgetary guidelines.
  • Represents the agency at community events, fairs, and expos.
  • Assists with the development and implementation of agency sponsored events.

Physician Relationships

  • Develops and maintains referral contact lists.
  • Documents weekly contacts and follow-ups.
  • Reviews potential referrals for additional service opportunities and recommends services.

Marketing

  • Creates personal marketing program for assigned territory.
  • Increases active client count, hours of home care services and number of patient referrals. Must be comfortable having a hospice conversation with families and patients.
  • Educates referral sources, patients and/or families regarding agency services available.
  • Meets or exceeds departmental productivity standards.

Education

  • Educates the general public on agency services via presentations, visits to organizations, group meetings and direct mailings.
  • Conducts meetings to educate referral sources on changes in Medicare/Medicaid standards that impact referrals to hospice and supportive services.
  • Assists in the development, implementation, revision and evaluation of departmental policy and procedures.
  • Notifies staff in writing of new community resources and updates the community resource book as new information becomes available.

Education and/or Experience

  • Associates or bachelor’s degree preferred.
  • Two years marketing experience preferred in hospice. Track record of success encompassing customer service, community relations, and physician relations preferred.

Position Summary

Performs consultation and admission visit to assess patient's needs and completed admission assessment and agreement documentation for VNA of NWI.

  • Possesses understanding of Hospice program and benefits.
  • Obtains orders for and procures immediate medications and needed DME/supplies.
  • Performs physical assessment of patients' condition, safety, knowledge, pain and symptoms.
  • Initiates Plan of Care.
  • Instructs on disease understanding, progression, and immediate interventions.
  • Documents assessment, current medications and initiates appropriate care plans.

Qualifications

  • Licensure: Must have and maintain license as a registered professional nurse by the state of Indiana.
  • Diploma must be from an approved school of professional nursing. Minimum of one year of medical/surgical experience in a hospital setting, nursing home, ECF, or Hospice Care preferred. Previous hospice experience and/or demonstrates understanding and support of the hospice philosophy and concept preferred. Must be sensitive to issues of loss and grief.
  • Must maintain current CPR certification.
  • Must hold and maintain a valid driver’s license, auto insurance, and professional liability.

RN Case Manager

  • Functions independently and appropriately, and demonstrates the ability to deal effectively with the demands of the job.

Position Summary

The Registered Professional Nurse serves as a case coordinator by providing, arranging for, teaching and supervising the health care of hospice patients in their home. The nurse appraises patient and family care needs and coordinates services with the interdisciplinary team.

Hospice Aide

Full-Time, Rotating Evenings/Night Shift for our 14 bed inpatient hospice center. Benefits offered.

Provides personal care to hospice patients at the VNA Hospice Center

Qualifications

  • Prefer minimum of 6 months as a CNA or HHA.
  • Current CNAs will receive training and certification as a HHA upon hire.
  • Must have and maintain valid driver's license and auto insurance.
  • Must maintain current CPR certification.
  • Must be computer literate.
