Hospice Aides (CNA) or (HHA)
Starting pay $18.50/hour
Full-time with on-call minimum rotation
Coverage Areas: Porter, LaPorte, Starke, Jasper, Newton and Lake County, mileage paid at the IRS rate.
Must be willing to go into patients homes and travel.

Clinical Liaison, RN

Position Summary
The Clinical Liaison is responsible for acting as a partner between the hospital and the agency by ensuring collaboration of care, coordinating discharge planning, and maintaining excellent communication. They will assist in developing relationships with physicians and hospital staff, meet with patients and families to educate them on VNA services, communicate with intake on the next steps, and ensure patients get the right service at the right time. The Nurse Liaison provides nursing care to patients who require GIP level of care in the hospital setting.
Qualifications
- RN with current registration to practice as a Registered Professional Nurse in Indiana
- Must hold and maintain valid Indiana driver's license.

Palliative Care Social Worker
$4,000 BONUS
Full-time (Monday -Friday, 8:00am-4:30pm)

Position Summary
VNA Hospice of NWI is looking for compassionate and empathetic Social Workers to work with our hospice, palliative & transition support patient needs. This role will involve providing care in the patients' homes and at Long Term Care Facilities.
Qualifications
- LCSW in the State of Indiana with at least 1 year of clinical experience in geriatric care, and/or medical systems.
- Have reliable transportation and willing to commute within a designated territory.
- COVID Vaccinated or qualified exemption.
- Strong interpersonal and written communication skills.
- Ability to be self-directed and work independently.
- Thorough knowledge of current computer software (ie. Word, Excel, 365).
- Ability to work within a team environment.
- The ability to work effectively in high-activity situations and under pressure.
Duties & Responsibilities
- Provide chronic disease management to hospice, palliative and/or patients needing support post facility discharge for transition support.
- Carries out psychosocial evaluations, including family dynamics, caregiver abilities and resiliency, communication patterns, neglect or abuse and plan intervention based on evaluation findings.
- Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
- Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record keeping system. Submits all clinical documentation, time records and other information in the required time frame.
- Works within a team concept with a nurse practitioner, nurse, chaplain and volunteer to support all of the psychosocial and medical needs of a patient and caregiver to promote independence, reduced hospitalizations, and self-determination in a patient-centered medical model.

Full-time, On-call RN Home Hospice
$5,000 - $10,000 BONUS (bonus based on FTE Status and experience)
Salary: $62,000.00 - $70,000.00 per year
Every other weekend Friday 4:30 pm- Monday 8:00 am on-call
Two alternating shifts during the weekday 4:30 pm - 8:00 am on-call

Nurse Practitioner-Hospice and Palliative Care (ACHPN Preferred)
Full Time Monday through Friday
Position Summary
Full-Time nurse practitioner needed in growing hospice and palliative care program. This position visits patients in their homes, facilities and our 13 bed in-patient center. Join a fast paced team with competitive pay and benefits. Additionally you will build meaningful relationships with the residents and their families.
Our nurse Practitioners collaborate with the Executive Director, Medical Director and Palliative Clinical Director to ensure that quality Palliative and Hospice Care is being provided to our patients.
Qualifications
- Current, unrestricted Nurse Practioner license in the State of Indiana
- A clinical background in adult, family or geriatric specialties
- DEA or ability to obtain
- Prescriptive authority or ability to obtain
- Minimum 1 year NP experience
- Certification in hospice and palliative care (ACHPN) (or willing to get certified)
- Ability to work autonomously
- Reliable transportation and insurance
- Ability to travel throughout our service area

- Possesses understanding of Hospice program and benefits.
- Obtains orders for and procures immediate medications and needed DME/supplies.
- Performs physical assessment of patients' condition, safety, knowledge, pain and symptoms.
- Initiates Plan of Care.
- Instructs on disease understanding, progression, and immediate interventions.
- Documents assessment, current medications and initiates appropriate care plans.
- Demonstrates comprehensive knowledge and skill in current nursing practice.
- Exhibits knowledge of the concept of a interdisciplinary team and a working knowledge of the development of plan for patient care and the ability to participate in them as well as using the case management approach.
