Hospice Social Worker MSW

Full-time with Minimum On-Call Rotation

Coverage Areas: Lake, Porter, La Porte, Jasper, Starke Counties, mileage paid at the IRS rate.

The VNA of NWI is seeking an MSW, Hospice Social Worker (full-time), minimum on-call requirements to provide direct supportive services to terminally ill patients and their families at our In-Patient Hospice Center and Patient Homes.

Position Summary

Works in conjunction with the Hospice interdisciplinary group (IDT) to provide direct supportive services to terminally ill patients and their families, and to consult with and support other agency personnel. The social worker will provide assessments of psychosocial problems and define goals and interventions to provide excellent end of life care.

  • Is a member of an interdisciplinary team (IDT) and provides social services in accordance with interdisciplinary plan of treatment.
  • Participates in the preparation, evaluation, and execution of the interdisciplinary plan of care to assure that the highest quality of care is given to those served.
  • Recognizes and utilizes community and family resources to assist the patient and family with end of life care needs.
  • Develops the plan of care with the IDT to deal with personal, financial, and environmental difficulties that may interfere with care.
  • Assists with funeral planning.
  • Keeps abreast of social work trends and knowledge.
  • Assists with funeral planning.
  • Participates in education both internally and externally to promote professional growth and understanding of hospice.
  • Maintains continuity of patient care by liaising with other health care professionals.
  • Attends IDT conferences and care plan meetings.
  • Arranges for extended care placement or respite placement when necessary.

Qualifications

  • Masters Degree from an accredited School of Social Work.
  • LCSW License Preferred.
  • Two years minimum experience as a medical social worker.
  • Knowledge of available community services/resources.

Job Duties

  • RN Case Manager will provide direct patient care to hospice patient case load.
  • Demonstrates comprehensive knowledge and skill in current nursing practice.
  • Exhibits knowledge of the concept of a interdisciplinary team and a working knowledge of the development of plan for patient care and the ability to participate in them as well as using the case management approach.
  • Functions independently and appropriately, and demonstrates the ability to deal effectively with the demands of the job.

Summary

The Registered Professional Nurse serves as a case coordinator by providing, arranging for, teaching and supervising the health care of hospice patients in their home. The nurse appraises patient and family care needs and coordinates services with the interdisciplinary team.
