Midnight RN

Sunday through Thursday 12:00AM - 8:00AM

Coverage Areas: Lake, Porter, La Porte, Jasper, Starke Counties, mileage paid at the IRS rate.

$2000.00 BONUS (bonus based on FTE Status).

The VNA of NWI is seeking a Midnight RN to work 8 hours shifts stationed at our In-Patient Hospice Center and Patient Homes with Possible QI Audits during down time.

Position Summary

Patient care visits, admissions, phone triage are all required. Hospice experience NOT required. Must be a registered nurse (RN)

Qualifications

  • Unrestricted RN license in the State of Indiana.
  • Excellent communication and organizational skills.
  • Minimum experience of 2 years medical surgical, critical care or oncology nursing.
  • Hospice experience preferred.

LPN

Full-time with Minimum On-Call Rotation

Coverage Areas: Lake, Porter, La Porte, Jasper, Starke Counties, mileage paid at the IRS rate.

The VNA of NWI is seeking an LPN to perform designated nursing functions under direction of the Registered Nurse.

$2000.00 BONUS.

Position Summary

The Licensed Practical Nurse performs designated nursing functions under the direction of the Registered Nurse. The activities of the LPN are based on his/her educational preparation and experience.

  • Assists the RN and/or physician in performing specialized duties related to end-of-life care.
  • Implements and continues the appropriate teaching in cooperation with the Registered Nurse.
  • Observes the patient for changes in their condition and reports these changes to the appropriate physician and registered nurse.
  • Records visits to patients per agency policy, including other pertinent information about patient and family.
  • Follows care plans as developed by the Registered Nurse and contributes data to the RN in the development of the plan of care and assists in carrying out the plan.

Qualifications

  • License and experience.
  • Graduate of a state accredited school of practical nursing.
  • Must hold and maintain current LPN license in the State of Indiana.
  • Evidence of experience in related health care field.
  • Must hold and maintain a valid driver’s license, auto insurance, and professional liability

Admission RN

Full-time with Minimum On-Call Rotation

The VNA of NWI is seeking an Admission RN (full-time), minimum on-call requirements to perform consultations and admission visits to assess patient's needs and complete admission assessments and agreement documentation for VNA of NWI.

$4000.00 BONUS.

Position Summary

Performs consultation and admission visit to assess patient's needs and completes admission assessment and agreement documentation for VNA of NWI.

  • Possesses understanding of Hospice program and benefits.
  • Obtains orders for and procures immediate medications and needed DME/supplies.
  • Performs physical assessment of patients' condition, safety, knowledge, pain and symptoms.
  • Initiates Plan of Care
  • Instructs on disease understanding, progression, and immediate interventions.
  • Documents assessment, current medications and initiates appropriate care plans.

Qualifications

  • Licensure: Must have and maintain license as a registered professional nurse by the state of Indiana.
  • Diploma must be from an approved school of professional nursing. Minimum of one year of medical/surgical experience in a hospital setting, nursing home, ECF, or hospic care preferred. Previous hospice experience and/or demonstrates understanding and support of the hospice philosophy and concept preferred. Must be sensitive to issues of loss and grief.
  • Must maintain current CPR certification.
  • Must hold and maintain a valid driver’s license, auto insurance, and professional liability.

Hospice Social Worker MSW

Part-time (15-20 hours per week) with Minimum On-Call Rotation

The VNA of NWI is seeking an MSW, Hospice Social Worker (part-time), minimum on-call requirements to provide direct supportive services to terminally ill patients and their families at our In-Patient Hospice Center.

Position Summary

Works in conjunction with the Hospice interdisciplinary group (IDT) to provide direct supportive services to terminally ill patients and their families, and to consult with and support other agency personnel. The social worker will provide assessments of psychosocial problems and define goals and interventions to provide excellent end of life care.

  • Is a member of an interdisciplinary team (IDT) and provides social services in accordance with interdisciplinary plan of treatment.
  • Participates in the preparation, evaluation, and execution of the interdisciplinary plan of care to assure that the highest quality of care is given to those served.
  • Recognizes and utilizes community and family resources to assist the patient and family with end of life care needs.
  • Develops the plan of care with the IDT to deal with personal, financial, and environmental difficulties that may interfere with care.
  • Assists with funeral planning.
  • Keeps abreast of social work trends and knowledge.
  • Participates in education both internally and externally to promote professional growth and understanding of hospice.
  • Maintains continuity of patient care by liaising with other health care professionals.
  • Attends IDT conferences and care plan meetings.
  • Arranges for extended care placement or respite placement when necessary.

Qualifications

  • Masters Degree from an accredited School of Social Work.
  • LCSW or LSW License Preferred.
  • Two years minimum experience as a medical social worker.
  • Knowledge of available community services/resources.

Job Duties

  • RN Case Manager will provide direct patient care to hospice patient case load.
  • Demonstrates comprehensive knowledge and skill in current nursing practice.
  • Exhibits knowledge of the concept of a interdisciplinary team and a working knowledge of the development of plan for patient care and the ability to participate in them as well as using the case management approach.
  • Functions independently and appropriately, and demonstrates the ability to deal effectively with the demands of the job.

Summary

The Registered Professional Nurse serves as a case coordinator by providing, arranging for, teaching and supervising the health care of hospice patients in their home. The nurse appraises patient and family care needs and coordinates services with the interdisciplinary team.
